Our clinic does a complete and thorough assessment of concussion using questionnaires such as Post-Concussion Symptom Scale, Rivermead Post-Concussion Guideline/Screening/ IMPACT Screening. Once questionnaires are completed, we start patient assessment by evaluating:
- Near point gaze, smooth pursuits, and saccades
- Vestibular ocular reflex
- Visual motion sensitivity and optokinetic reflexes
- Gait evaluation/tandem/Rhomberg’s test
- and a series of Balance tests
With this comprehensive examination we try to identify the etiology of the concussion, namely, from a cervicogenic standpoint to a vestibular ocular and oculomotor perspective to an autonomic dysfunction.
We evaluate autonomic dysfunction using the Buffalo Concussion Treadmill Test. Once we have identified the cause of the concussion, we develop strategies and apply them through different phases of rehabilitation.
In phase 1 we gear rehab towards: special awareness, stability of fixation, postural control visual tracking (pursuits, saccades) and simple vestibular ocular reflex retraining.
In phase 2, we may add: balance, gait, and dual tasks, performing visual motion sensitivity/ optokinetic ex., eye teaming and convergence.
In phase 3 we may increase: complexity of the environment, integrating more tasks and adding cognitive tasks.
Through this phased implementation of rehabilitation, concussion patients are able to make significant progress in symptoms such as balance, posture, vertigo, migraines, and activities of daily living.
